A sleep tracking tool

Hello,

Would it be possible to track my sleep over the days? Bedtime and wake-up time (and calculation of sleep duration), night-time awakenings, sleep quality, possible addition of notes (if insomnia or difficult waking etc.). Do I already wake up exhausted in the morning?

Ideally, I'd like to have these data graphed, or even to be able to correlate them with the treatments I'm taking.
